Registration Fees (In USD): $750 for all attendees. All registration fees must be received prior to the seminar. You may pay by credit card, by check payable to "captive.com, LLC," or by wire transfer. Payments by check must be received at the Captive Workshops office no later than July 6. Dress Code: Business Casual -- relax and learn! Cancellation Policy: Seminar and Room cancellations must be made in writing to the Captive Workshops office. Cancellations on or before July 6 will receive a refund of 50% of the total seminar and room fees. Cancellations after July 6th will not receive a refund; however, we would be happy to transfer your registration and room reservation to another member of your company or to a colleague. If Captive Workshops cancels the event, Captive Workshops is not responsible for any airfare or other costs incurred by registrants. Registration options: Register on-line on our secure server. This option allow you to register for the seminar AND reserve your room, if space is still available. If you prefer to register by manually downloading faxable forms: Download our fax reservation form, complete and fax it to Chris Mancini at 860.276.9795, along with your payment instructions. If you registered for the seminar by fax, be sure to complete your hotel reservation.
